STEP 1: INITIAL CONNECTION

Give me an idea of the issue(s) you’re dealing with, and I’ll let you know if I can help you. We can discuss any questions you might have at that time and you can get a feel for whether you think I’d be a match for what you’re looking for. At no cost to you.

STEP 2: CLIENT PROFILE

Before our first session, I’d like get your backstory and some info about your presenting issue. To make this easy, save time in session and save you some money, download, print off and complete and send back a copy of this Individual Intake Form OR Couple Intake Form.

If possible, I would like to have at least 24 hours before our first meeting to review your Intake Form. If you don’t have access to a printer, please arrive at my office at least 20 minutes prior to our scheduled start time to complete a printed copy of the Intake Form. Simply request a copy and pen from one of the reception team and fill it out prior to our first meeting.

Once filled out, either (in order of preference),

1. Scan and email to drpaulday@gmail.com
2. Send by mail
3. Bring completed form with you to our first session

STEP 3: INFORMED CONSENT

Before we formalize our clinical relationship, it is necessary for you to download, print off, read and sign the Informed Consent. If you have any questions, hold off signing and let’s discuss your questions or concerns. Once signed, either (in order of preference),

2. Send by mail
3. Bring the signed document with you to our first session

STEP 4: FINANCIAL AGREEMENT

To formalize our agreement, please download, fill out and sign a copy of the Financial Agreement.

STEP 5: SERVICES AND FEES

Billable Services

1. In-Office Session: a face-to-face meeting in my office or other location
2. Video Session: a video conference meeting using Skype, Gmail video, Oovoo or other
3. Telephone Session: an audio meeting via the telephone
4. Email Session: a single exchange where client sends message and gets one response
5. Brief Call): a brief telephone or video contact that is less than 10 minutes
6. Assessment: 16PF, MBTI, Enneagram, Prepare-Enrich, DASS, Bischof Symptom Inventory, Mental Health Inventory
7. Auxiliary Services: includes calls or research made on behalf of the client at their request
8. Reports: client requested reports for benefit or insurance provider, courts, etc
9. Notes: photocopy of confidential notes

Financial Policies

1. Late Cancel, No Rebook (LCNR): less than 24 hr notice, no rebook within 2 business days
2. Late Cancel, Rebook (LCR): less than 24 hr notice, rebooks within 2 business days
3. No Show: client does not show for appointment

STEP 6: : SELECT DATE AND TIME

Below is a list of the days and hours I am in the office for face to face meetings. Select a day and time that would work best for you and let connect to see if that time is available. As these time slots fill up, I will expand my office hours. If you are unable to meet during these times, contact me and propose an alternative date and time. (NOTE: Effective September 1, 2017)
